Mindset Programme – Professional Referral

Our MindSet Programme offers social activity and peer support groups with a focus on mental health and wellbeing for adults aged 18 and over.
If you are a professional, you can download our referral form below. Please return the form to the email address on the form.

Due to the high number of referrals we are currently receiving, we aim to respond with 28 days. In the meantime, you are more than welcome to visit the centre with your client, and spend time in our community café and/or garden to get a feel for the environment.
